You can use SurveyMonkey free basic account with some limitations: a maximum of 100 respondents, no more than 10 questions per survey, data in PDF only. See here for a list of survey templates.
QuestionPro
You can create unlimited surveys and polls with QuestionPro free account but it is limit to 100 responses per survey. See here for the different survey templates.
Google Form
Google form allows you freely custom forms for surveys and questionnaires and gathers the responses in a spreadsheet. You can select the different question types, drag-and-drop to reorder questions, set data validation for input fields and customises values as easily as pasting a list.
JotForm
A free form builder with basic survey features like scale rating, matrix, etc. With a free account, you can use JotForm fully up to 100 submissions per month.

Tabula
Tabula is a free tool that allows you to extract PDF data to CSV format. It works best with Google Chrome and Firefox. Please note that Tabula only works on text-based PDFs at this time, not scanned documents.
